Retrospective analysis of the development of the organization of medical and social work with persons with disabilities in foreign countries

Abstract
This article presents a retrospective analysis of the formation and evolution of the organization of medical and social work with persons with disabilities in foreign societies. The study covers key historical stages in the development of assistance models — from antiquity to the present — and traces the transformation of approaches from religious and philanthropic practices to state-regulated and inclusive systems. A comparative-historical and systemic approach was applied, based on the analysis of academic literature, international legal frameworks, and practical models. Special attention is paid to the biopsychosocial model as the contemporary foundation for interdisciplinary disability support. The analysis highlights institutional development trends, the strengthening of legal frameworks for persons with disabilities, and the emergence of integrated rehabilitation strategies. The results of the study may be applied to enhance the Russian system of medical and social support through the adaptation of international best practices.

Utilizing large language models in medical product market authorization dossier preparation

Abstract
Large Language Models (LLM) have emerged as powerful tools in various industries, promising to revolutionize processes through their advanced natural language processing capabilities. In the context of medical product market authorization dossier preparation, LLM offer the potential to streamline workflows, enhance efficiency, and improve compliance with regulatory standards. However, there are a number of obstacles to their adoption, including those pertaining to data security, output reliability, regulatory compliance, and the requirement for a strong infrastructure and qualified staff. This article explores the challenges and perspectives surrounding LLM usage in medical product dossier preparation, highlighting the importance of proactive measures, collaboration, and innovation in harnessing the full potential of LLM. Through a literature review, expert interviews, case studies, and analysis of regulatory guidelines, common themes, best practices, and potential solutions are identified. The findings underscore the critical role of collaboration among industry stakeholders, knowledge sharing, and the establishment of dedicated forums or consortia in overcoming barriers and driving collective innovation. Organizations can achieve efficient, compliant, and timely market authorization of medical goods, thereby enhancing patient outcomes and benefiting public health, by properly resolving hurdles and capitalizing on the benefits of LLM technology.

Digital competencies of a medical professional: a modern standard of professionalism

Abstract
In the context of the digital transformation of healthcare, medical personnel are faced with the need to master digital competencies that ensure sustainable professional viability in a new clinical and organizational environment. The article presents a theoretical and analytical justification for the need to develop digital literacy of medical professionals, reveals the structure of digital competencies, including technical, informational, communicative, legal and reflexive components. The key reasons for the demand for digital skills in professional activities are analyzed: the implementation of the state policy of digitalization of healthcare, the growth of clinical information, the development of telemedicine, and the increase in digital activity of patients. Based on the analysis of statistical, regulatory and empirical sources, conclusions are formulated about the need for the institutional implementation of digital competencies in the system of training and support of medical personnel.

The need to assess and expediency of attracting non-medical professionals to medical organizations

Abstract
The work of medical organizations, management processes and implemented formats of medical care are undergoing significant transformations related to both medical care technologies and new digital capabilities of the Ministry of Health. This determines, on the one hand, an increase in the requirements for the possession of IT competencies by doctors, and on the other hand, an increase in the "non-core" burden on medical personnel. That is why the involvement of "non-doctors" and IT specialists should compensate for the needs of medical organizations in this area. However, it is very problematic to accurately determine the number of specialists without medical education who perform related functions in the provision of medical care. One of the possible approaches to solve this problem is the implementation of an expert approach structured in the form of a SWOT analysis with a weighted score. This will allow us to identify all favorable and promising opportunities for attracting "non-medics" to the Ministry of Defense, as well as to assess the difficulties and associated risks of this attraction.

Modern aspects of the problem of burnout of medical workers

Abstract
Technological advances in the field of medicine, which received an additional impetus to development during the COVID-19 pandemic, and subsequent organizational changes, increased requirements for high efficiency and quality of work of medical organizations, often continue to have a significant impact on the well-being of medical workers even after the pandemic in the absence of sound management. The work environment is a key factor determining the well-being and health of medical workers. Increasing the competence of managers in determining strategies for preventive solutions to the problem of an unfavorable working environment is the main condition for reducing the spread of professional burnout syndrome and personnel shortages in medicine. A well-structured work environment, organizational and communicative cultures of medical organizations and improved working conditions of medical workers are significant determinants of the effectiveness and quality of healthcare.

The impact of mentoring on the professional satisfaction of professionals

Abstract
Mentoring in healthcare is becoming an increasingly relevant topic, especially in the context of continuous professional development and changes in the healthcare system. The article examines the role of mentoring in the field of medicine as an important tool aimed at improving the professional training and support of new medical professionals in the face of rapid changes in the technological and organizational aspects of healthcare. The study is based on quantitative and qualitative analysis methods, including surveys and in-depth interviews conducted among medical professionals in Moscow and the Moscow Region. The results showed that mentoring significantly affects professional satisfaction, contributing to the harmonization of work processes and emotional support for employees. The author recommends that medical institutions develop and structure mentoring programs, taking into account the revealed survey results, which will not only improve the quality of medical care, but also improve the general condition of healthcare workers, thereby contributing to the effective adaptation and professional growth of new specialists. It is noted that further research in this area will be equally important to understand the long-term effects of mentoring and its impact on the entire healthcare system.

On the prevalence of contact dermatitis among health workers in different countries

Abstract
It is known that contact dermatitis is more common among healthcare workers than among the general population (21% versus 10%, respectively). The prevalence of occupational contact dermatitis among healthcare workers in recent years has ranged from 12% to 65%. The average prevalence of contact dermatitis of the hands among healthcare workers has increased to date from 21.08% to 37.24% due to the COVID-19 pandemic. This pathology, as one of the most common occupational skin diseases and a socially significant health problem, can lead to loss of productivity, sick leave, decreased quality of work and life, and decreased job satisfaction. In light of the global shortage of medical personnel, issues of occupational safety, monitoring of skin condition, and surveying healthcare workers about skin pathologies before hiring are relevant.

Lev Fedorovich Zmeev (1832—1901)

Abstract
The article is devoted to the prominent Russian historian of medicine and bibliographer, Doctor of Medicine Lev F. Zmeev (1832—1901), the author of bibliographic dictionaries and monographs on the history of medicine of the 16th — 19th centuries, in which he emphasised the originality of the development of the Russian medical profession. Lev F.Zmev worked in the Caucasian Mineral Waters Administration, in the Samara and Voronezh zemstvos, in the Medical Department of the Ministry of Internal Affairs. He served in hospitals during the Russian-Turkish war (1877—1878). He lectured on the history of medicine in Russia at the Military Medical Academy. He was also one of the organizers of societies of Russian doctors in Pyatigorsk, Moscow, St. Petersburg.

Analysis of the assortment of medicinal products containing glucocorticosteroids for the pharmacotherapy of atopic dermatitis in pediatrics

Abstract
In the last decade, there has been a steady upward trend in the incidence of atopic dermatitis, a genetically determined inflammatory skin disease, among the child population. The inclusion of topical glucocorticosteroids in pediatric practice is justified, since these drugs control the symptoms of dermatological disease, reduce the risk of severe course of the pathological process. In the course of the work, a list of glucocorticosteroids for topical use presented in pharmacies of the metropolis was studied. An assessment of the range of drugs in this group revealed that the proposed drugs have a variety of release forms and a wide price range. A study of the ratio of imported and domestic drugs revealed that the main country producing the analyzed drugs is Russia. However, in the list of these pharmacies at the time of the study, an insufficient range of concentrations (weak, moderate or strong activity) of topical glucocorticosteroids was noted, indicated in clinical guidelines for the treatment of atopic dermatitis in children. Thus, it has been established that it is necessary to expand the range of glucocorticosteroids for topical use for pharmacotherapy of atopic dermatitis in children of different age groups.

Facets of scientific and practical activities of the honored doctor of the Russian Federation, professor S. I. Stegunin

Abstract
The article is devoted to the creative heritage of the Russian social hygienist, organizer of healthcare and higher medical school, head of department of Samara State Medical University Sergei Ivanovich Stegunin (16.05.1923—07.04.2004). The leading scientific areas of research of Professor S. I. Stegunin and his school were topical issues of the theory and history of social hygiene and organization of healthcare, epidemiology of the most important non-communicable diseases, improvement of outpatient and inpatient care. S. I. Stegunin substantiated and applied the method of multivariate analysis to study the influence of a complex of risk factors on the occurrence and development of non-communicable diseases. The results of his research contributed to the deepening of the scientific and practical base of preventive medicine and medical examination. The article highlights the versatile achievements of Professor S. I. Stegunin as a teacher, scientist, organizer of healthcare, editor, public figure, popularizer of medical knowledge and publicist.

Risk analysis for the quality of medicines of pharmaceutical manufacturing using a burette unit

In the article, research of the risks to the quality of medicines manufactured in pharmacies that may arise when using a burette unit is carried out. The risks are evaluated using the Ishikawa diagram method. As a result, it was found that due to a significant number of flaws when using the burette unit, its use is currently not advisable.

Portable oxygen sources used for long-term home therapy of patients with COPD

Abstract
The article discusses medical devices registered on the domestic pharmaceutical market that are used in long-term home oxygen therapy for patients with chronic obstructive pulmonary disease (COPD). The aim of the study was to investigate portable oxygen sources used in patients with COPD at home. The methods of statistical, logical, comparative and content analysis were used in the study. The relevance of studying the portable oxygen sources market is due to an increase in the number of patients with chronic obstructive pulmonary disease who require long-term oxygen therapy at home. According to the nomenclature classification of medical devices and codes of types of medical devices, portable oxygen sources belong to the group of "Anesthetic and respiratory medical devices". As a result of studying the technical characteristics of various medical devices, we proposed to classify them as portable oxygen sources: a portable liquid oxygen unit, a mobile/portable oxygen concentrator, artificial lung ventilation devices with the possibility of non-invasive administration, inhaler cans with a gas oxygen mixture. In the State Register of Medical Devices, electric ventilators, constant positive pressure ventilators for home use and mobile/portable oxygen concentrators are represented in the largest number of valid registration certificates. The most significant increase in registered medical devices used in home oxygen therapy was noted first in 2020 and then in 2024, which we attribute to the coronavirus pandemic and its consequences. Portable oxygen sources from foreign manufacturers prevail in the Russian pharmaceutical market (87%), China is the leader (18 manufacturers), followed by the USA and Germany (14 manufacturers each). In the Russian Federation, patients suffering from severe respiratory diseases have the right to receive medical devices and medical equipment free of charge as part of palliative care provided at home, including portable oxygen sources.

Changes in the positions of domestic manufacturers in the regional market because of offset contracts’ supplies of medicinal products

Abstract
Pharmaceutical production is the leader in concluding contracts with counter investment obligations (offset contracts) among all industries. The bulk of contracts are concluded in Moscow, the largest regional market of medicinal products. The objective of the study was to determine the trends in structural changes in the Moscow pharmaceutical market after the start of deliveries under all offset contracts concluded at the beginning of April 2024. The study was based on the information from the official website of the Unified Information System in the Sphere of Procurement and data by DSM Group on medicinal products sales. The volume and structure of the market in the base period (2021) was compared with average annual supply figures under concluded offset contracts. In the period under review, 8 offset contracts were concluded in Moscow for the supply of drugs from 139 INN groups. In the base period the total supplies volume for 139 INNs under the regional programs of preferential drug provision, which are the focus of the offset contracts’ supplies, amounted to 13.4 billion rubles. According to the calculation model, as the result of offset contracts’ supplies, the total share of Russian manufacturers in the market of 139 INNs in the segment of the regional programs may increase from 16.6% to 63.6%, in the total government procurement market — from 23.5% to 45.8%, in the entire Moscow market (including retail sales) — from 18.6% to 31%. It was shown that already in the base period the share of Russian manufacturers was more than 90% in the segments of 24% INN, and more than 50% — in segments of 33% INN. In this regard, the advisability of further producers’ stimulation looks questionable, if we mean not attracting investments in the development of production in a specific region but achieving the goals of import substitution and maintaining competition in the public procurements of medicinal products in the all-Russian market. As a result of the implementation of the offset contracts’ supplies, with consideration of specific INN segments, a significant increase in domestic manufacturers’ share of the Moscow pharmaceutical market can be expected. At the same time, offset contracts’ supplies limit competition in the public procurement system with respect to other Russian manufacturers and create additional competitive advantages for attracting investment in Moscow preventing of the development of other regional pharmaceutical clusters.

Identification of predisposition to the development of arterial hypertension in students in the study of indicators of systemic hemodynamics

Abstract
The identification of a predisposition in young people to the development of arterial hypertension (AH) is an urgent task of preventive medicine, since the formation of hypertension does not begin at 40—50 years old, as previously thought, but at an earlier age. The article presents the results of a study of systemic hemodynamics by volumetric compression oscillometry (VCO) in students of the Medical Faculty of the State University of Education. In the examination of 191 students aged 18—25, normal blood pressure was noted in 78.5% of cases, a downward trend in blood pressure was detected in 19.4% of the subjects and 2.1% of the subjects had elevated blood pressure. The determination of the main indicators of hemodynamics by the VCO method made it possible to correlate each respondent with a certain profile. Among the examined group, 16 hemodynamic profiles were identified. The hemodynamic profile 222, corresponding to the normal values of heart rate, systemic vascular resistance and stroke volume, was noted in this group only in 31.4% of respondents. The discrepancy in this group of subjects in the number of detected cases with normal blood pressure values to the number of detected cases with normal values of key hemodynamic constants proves the need for early prevention of hypertension, allows identifying a risk group for hypertension, prescribing personalized preventive measures and optimizing preventive medical examination.

Information strategies for increasing adherence to vaccination in the primary health care system

Abstract
Increasing the population's commitment to vaccination is a key task of the modern healthcare system, especially at the primary health care (PHC) level. Refusal of vaccinations or a low level of confidence in vaccine prevention threatens the sustainability of epidemiological well-being. The article analyzes modern information strategies aimed at increasing vaccination coverage, taking into account international and domestic experience. The most effective approaches to organizing information work have been identified, and the factors influencing the formation of behavioral attitudes towards vaccination have been identified.

Innovative development of China’s pharmaceutical industry: a systematic review of foreign scientific literature

Abstract
This article presents a systematic review of foreign scientific literature dedicated to the innovative development of China’s pharmaceutical industry. It examines key factors influencing the formation and realization of companies’ innovation potential, including the role of government subsidies, tax benefits, regulatory policies, and mechanisms of interaction with research institutes. Particular attention is paid to issues of industry fragmentation, regional differences in the effective use of innovation resources, and the need to refine the legal framework for intellectual property protection. The findings demonstrate a growing interest in developing original drugs, driven by heightened domestic market competition and state-led incentive measures. These conclusions and recommendations may be useful for shaping policies and practices aimed at managing innovative development in the Russian pharmaceutical sector.

The market of public-private partnership projects in the healthcare sector in the Russian Federation: structure and trends

Abstract
The aim of the study was to determine the structure and trends of the public-private partnership projects market in the healthcare sector in the Russian Federation. The information and analytical base of the study was data on investment projects implemented on PPP principles and included in the register of the Rosinfra digital platform. Qualitative and quantitative research methods were used. The PPP market in the healthcare sector has been formed and is developing in different areas of industry activity. However, the analysis of the structure and trends showed its imbalance in models, legal forms, and areas of MP. Conditions have not been created for investors in socially significant areas — primary care, medical rehabilitation. The analysis of the structure and trends of the PPP market in the healthcare sector showed that it is necessary to develop mechanisms for attracting investments in the primary care (general therapeutic area) and improve special legislation in terms of PPP agreements in order to replicate infrastructure models in the industry. This will lead to a decrease in the number of low-capital-intensive and short-term projects, which will contribute to more sustainable development of PPP in the healthcare sector.

Modern view on the mechanisms of therapeutic effect of homeopathy

Abstract
The article presents an overview of the results of research into the mechanisms of therapeutic effects of homeopathic medicines, the disclosure and substantiation of which allows us to talk about homeopathy as one of the scientific directions of modern medicine. The article gives a historical excursus of homeopathy development from ancient medicine to the present day. It is emphasized that for more than 200 years of its existence not only clinical, but also research experience in the application of this method has been accumulated. The authors consider scientific concepts, hypotheses and methodological approaches aimed at explaining possible mechanisms of action of homeopathic medicines on the human body.
