Accessibility of medical care for adult cancer diseases using off-label drugs: clinical recommendations and standards of medical care analysis

Abstract
Currently, the use of off-label drugs is an actual problem all over the world. Use of off-label drugs are not fully regulated in Russian legal system. While the study, clinical guidelines for the treatment of cancer for the adult age group posted in the Rubricator of the Ministry of Health of Russia were analyzed for the presence of off-label anticancer drugs and an assessment was made of the consistency of the lists of anticancer drugs in clinical guidelines and standards of medical care. The results of the study showed that a number of clinical guidelines for the treatment of cancer describe the use of off-label drugs. However, due to legislative restrictions, these drugs are not included in the standards of medical care for adults with cancer developed on the basis of clinical guidelines. This creates a situation that leads to inconsistency between the standards of medical care for adults and clinical recommendations. For the healthcare system, the issue of regulating the procedure for including off-label drugs in the standards of medical care for adults on the basis of evidence-based information presented in clinical guidelines is an urgent issue in order to harmonize documents that regulate both the provision and financing of medical care in the healthcare system of the Russian Federation to improve its accessibility.

The role of the personal brand of medical workers in the formation of demand for medical services

Abstract
The purpose of this study is to consider the influence of a doctor's personal brand in the formation of demand in the medical services market. The article discusses the issues of marketing in medicine, the influence of the personal brand of medical workers on the demand for specialists, patient confidence and the formation of demand for medical services. The socio-economic, psychological, cultural and ethical changes taking place in modern society force us to transform the usual approaches and practices both in marketing and in the organization of business processes in the field of medical services, causing the relevance of the research topic in conditions of high competition. Patient trust is an important and relevant issue in modern medicine and a key factor in doctor-patient interaction. Therefore, it is very important for a doctor to be able to win the patient over and gain his trust, which is being solved today with the help of a doctor's personal brand. In conclusion, it is concluded that the growth of a doctor in all respects is helped by painstaking work on a personal brand, which forms the demand for medical services.

Abstract
The health of employees is currently considered by companies as a resource that allows them to ensure the growth of productivity, efficiency and competitiveness. It is well known that office workers spend most of their working time in a sedentary state, which contributes to a sedentary lifestyle that increases the risk of developing diseases and disabilities. The gradual decline in cardiorespiratory fitness among adults, along with an increase in the number of non-communicable diseases in developed countries, makes the workplace an important opportunity to promote a healthy lifestyle. Progressive, well-managed, responsible companies are ready to implement employee wellness programs within the framework of corporate social responsibility. Companies have opportunities to develop strategies to encourage employees to take a responsible attitude to their health at the workplace and in the process of performing their official duties.

The article discusses modern innovative technologies and digital products (platforms and applications for smartphones) that can significantly improve the efficiency of emergency (including emergency) medical care. The following areas of technological improvement are highlighted, such as the use of artificial intelligence in sorting patients, the use of applications to optimize the process of emergency care, the use of portable diagnostic devices at the place of medical care, the use of medical drones to provide assistance from the air, systems for simplifying transportation and the use of unmanned ambulances, as well as equipping ambulance vehicles.

Views about the nature of skin diseases in children and their treatment in Russian folk medicine

Abstract
The article attempted to reveal the specifics of ideas about skin diseases in children, their causes and methods of treatment in Russian national everyday medicine. Peasant culture of the XIX — early XX centuries was based on medieval models, which led to an appeal to the published historical, ethnographic and folklore sources of this time. Doctor books with sections dedicated to children stored in the Russian National Library in St. Petersburg were also analyzed. The study of materials allows you to draw a conclusion that in Russian folk medicine there has long been a variety of views on the causes of skin diseases and their treatment in children, based on various interweavings of rational and irrational ideas, on combinations of Christian and pagan views.

«Polyclinic. Reboot» — a unified standard for the organization of primary health care in the Moscow region in outpatient settings

Abstract
Primary health care is the most widespread type of medical care. Improving the PHC organization system is a strategic priority for the development of the healthcare system at the regional level, since the impact of PHC on the effectiveness of the entire healthcare system is well known. The Ministry of Health of the Moscow Region summarized and systematized proposals for improving the main processes of primary care in outpatient settings, formed on the basis of experience in the use of lean manufacturing technologies, and introduced organizational and management technology that allows optimizing the management of the regional system of primary health care in outpatient settings — the standard of organization of outpatient care in the Moscow Region (Standard). The standard regulating uniform approaches to the organization of polyclinic activities includes six areas characterizing the main processes of organizing the work of the structural units of the polyclinic (distribution of patient flows, standards of prevention and medical examination, etc.), requirements for employees, equipment and location of offices (registry, diagnostic and treatment rooms,) and, thus, the creation of comfortable conditions for patients (location of offices on the first floor, information, navigation, etc.).

Staff and regulatory support for the profile «psychiatry-narcology»

Abstract
The article provides an analysis of the labor standards specified in the order of the Ministry of Health of the Russian Federation on the Procedure for providing medical care in the profile «psychiatry-narcology», contradictions between different applications of the document in terms of the normative population size for the establishment of positions of psychiatrists-narcologists are revealed, erroneous positions on the recommended number of positions to ensure round-the-clock work are considered. An important conclusion of the publication is the need to solve the identified problems not only by making changes to a specific document, but in general in creating a labor rationing system that has been destroyed in recent years.

Proactive medical and social patronage in the system of long-term care (literature review)

Abstract
The article presents an analysis of the global long-term care market. The place and role of proactive medical and social patronage in the healthcare and social services systems of the Russian Federation are reflected, aspects of its regulation are presented, as well as its contribution to the achievement of sustainable development goals in the Russian Federation. The features of interdepartmental interaction in the implementation of proactive medical and social patronage in the Russian Federation, including in the city of Moscow, are given. IT solutions for providing proactive medical and social patronage in the countries of the world are considered. It was revealed that significant steps are being taken in the world for the widespread introduction and scaling of successful long-term care practices, the most important of which in the Russian Federation is proactive medical and social patronage, but the professional community has yet to work out a significant pool of issues related to this type of service.

Eye diseases in children in the Republic of Sakha (Yakutia): clinical and epidemiological characteristics

Abstract
The article presents an analysis of the indicators of the general morbidity of children and adolescents with diseases of the eye and its accessory apparatus, analyzes the results of in-depth medical examinations by ophthalmologists of schoolchildren in Yakutsk of the Republic of Sakha (Yakutia). During the study period, a slight increase in the overall incidence of eye diseases and appendage apparatus in adolescents was shown, an increase in the overall incidence of myopia and astigmatism in children and adolescents, and a high proportion of children and adolescents with eye diseases in Yakutsk was revealed according to the target medical examination. The results obtained once again confirm the population significance of this pathology in children and adolescents, which will allow us to revise the organization of ophthalmological care in the region, taking into account the possibilities of correction.

Characteristics of the structure of arriving foreign citizens in the Republic of Tatarstan

Abstract
Annotation. Introduction. According to the requirements of legal documents, foreign persons upon arrival in the country within a strictly allotted time must undergo a medical examination on the basis of organizations licensed for this type of activity and be authorized to conduct a mandatory medical examination of foreign citizens. In Kazan, on the basis of the dermatovenerological dispensary, in order to conduct a high-quality and prompt medical examination of foreign citizens, a department was organized, operating in the «single window» mode, upon contacting which foreign citizens can obtain a medical opinion based on the results of laboratory diagnostic tests, examinations by medical specialists. The aim of our research is characteristics of the structure of arriving foreign citizens in the Republic of Tatarstan based on data on foreign citizens who underwent a medical examination in the dermatovenerological dispensary of the city of Kazan for 2018—2022. Materials and methods. In the course of the study, a comparative analysis of the normative documents regulating the issues of medical examination of foreign citizens was carried out. Data from the state information (by geographical principles, age, directions and countries from which the converts originated) system «Electronic Health of the Republic of Tatarstan» on foreign citizens who underwent a medical examination in Kazan for 2018—2022 were analyzed on the basis of the State Autonomous Healthcare Institution «Republican Clinical Dermatovenerological Dispensary of the Ministry of Health of the Republic of Tatarstan named after Professor A. G. Ge. The analysis of the structure of arrived foreign citizens by gender, age, purpose of arrival and countries of origin of the applicants was carried out. The data were processed using Microsoft Excel (USA) and StatTech (Russia). Results. Despite the COVID-19 pandemic, the annual flow of foreign citizens to the Republic of Tajikistan did not stop: in 2018—2022. On the basis of the dermatovenerological dispensary of the city of Kazan, 38,179 people underwent a medical examination, of which 78% were men. The predominant age groups among both women (n = 8539) and men (n = 29,640) over a 5-year period were 18—29 years old (55% and 53% respectively) and 30—39 years old (16% and 24% respectively). A significant proportion of foreign citizens who arrived were labor migrants (32%), students (26%) and persons applying for a residence permit (19%), 21% of persons did not indicate the purpose of their arrival, refugees accounted for only 2%. Labor migrants and persons applying for residence permits were mainly representatives from Uzbekistan and Tajikistan. Students came from Turkmenistan 17.8%, China 13.2%, Egypt 10.6%, Iran 7.9%, Kazakhstan 7.4%, India 7.3% and other countries. In 2022, there is a sharp increase (by 130 times) in the number of students (9947 people), compared to 2021 (76 people) and previous years, which is associated with the introduction of additions to the legal documentation of the requirements for mandatory medical examination by students.

Improving the activities of private medical organizations under the influence of modern trends in the development of healthcare

Abstract
The article examines the state of the private medical services market at the present stage and examines the problems that stand in the way of the development of the commercial sector of the medical services market. The authors considers the prospects for the development of private medical organizations in new economic and geopolitical conditions. The economic transformations currently taking place in the life of our country are reflected in the development of the private medical services market, becoming a test in the activities of commercial medical organizations. Over the past two years, all business structures have been adapting to anti-Russian sanctions and the consequences of restrictive measures in connection with the coronavirus pandemic. Therefore, these two areas of problems — establishing partnerships to provide materials and equipment with friendly countries and eliminating the consequences of the coronavirus pandemic — are significant for the development of private medicine in modern conditions. The state of public health and public health are the main indicators characterizing the level of quality of life of the population of the country, the socio-economic development of the state and its position in the world economy. Therefore, the solution of health problems is an urgent topic that is actively discussed by experts and participants in the medical services market. In conclusion, it is concluded that in the long term, the private medical services market will be represented by a moderate number of multidisciplinary medical centers in the premium price segment and a large number of highly specialized medical clinics. The purpose of this study is to consider modern problems and prospects for the development of private medical organizations.

Topical issues of the functioning of the private healthcare sector in Russia

Abstract
Today, private medical clinics are not uncommon. In every city and almost every locality there are private clinics that provide various kinds of paid medical services, ready to receive patients at any time. The article presents an analysis of the number of licensed private medical clinics in the Russian Federation in the context of federal districts with an emphasis on the Southern Federal District. In addition, in order to identify consumer preferences in the market of medical services provided by private medical clinics over public ones, the author conducted an anonymous questionnaire survey, which was attended by 434 respondents living in the Krasnodar Territory. The respondents' preferences when choosing a medical service are determined, the degree of satisfaction from admission to a private or public medical organization is revealed. The conclusion is made about the high degree of customer orientation of private medical organizations.

The opinion of pharmaceutical workers in the city of Voronezh on the benefits of conducting information work when dispensing drugs for pharmacy organizations and consumers was studied. Experts consider that it is necessary to further improve the algorithms for counseling and informing. Also experts believe that one of the topical areas of development is the use of personalized medicine approaches to optimize drug therapy. Requirements for professional growth, providing qualified assistance to customers, updating the documentation of standard operating procedures determine the importance of increasing the level of knowledge of pharmaceutical workers in this sphere.

The development of the Russian pharmaceutical market at the beginning of the XXI century. Price trends and key factors.

The article presents a review of the development of the Russian pharmaceutical market over the past few years in the context of price trends. Analyzed market changes in the crisis periods of Russia's economic: 2008—2009, 2014—2015, 2019—2021, 2022. Their influence is shown on the price of medicines, analyzed an average price's changes and the inflation component. The analysis was carried out in the context of different market segments (prescription and OTC medicines, retail and hospital segment, EDL drugs). The reasons that led to these changes are described.
