The article examines current issues in organizing the procurement of medical materiel in health care organizations within the system of the Ministry of Defense of the Russian Federation. It analyzes specific challenges arising from dual regulation, planning under budgetary constraints, and stringent mobilization-readiness requirements. The article identifies key problem areas: low operational efficiency; a persistent gap between planning and the actual needs of medical units; and reliance on outdated performance metrics. It proposes a comprehensive optimization model that includes diagnostic assessment, process reengineering with elements of automation, the adoption of modern managerial approaches — category management, total cost of ownership, and balanced scorecard — and accompanying organizational change. It presents the economic and strategic rationale for transforming the procurement department from a cost center into a strategic partner that ensures timely, accurate, and uninterrupted supply of critical items, thereby supporting the continuous functioning of military medicine.
